To get your residential mortgage loan or home equity line started
at The PrivateBank, please consider the following checklist.
Mortgage Checklist
- Mortgage Application
- Pay Stubs: Copies of the two most recent months
- W-2 Forms: Copies of the two most recent years Please Note
The Following:
- Self-Employed: Include copies of your personal Federal
Income Tax Returns for the two previous years including all
schedules and statements, K-1s for all partners, and all
corresponding W-2s. Please provide original signatures on
all copies.
- General Partner: If you hold more than 25% interest as
a general partner, please submit signed copies of your Federal
Partnership Income Returns for the previous two years.
- Limited Partner or Corporation: If you own 25% or more
of the outstanding capital stock of a company, please submit
copies of your Federal Corporation Income Tax Returns for
the previous two years. Also include the Corporation Financial
Statements for the most recent year.
- Asset Statements: Copy of the most recent bank statements (checking,
savings, securities, money market accounts, IRAs, 401(k) pension
plans)
- Homeowner's Insurance: Agent's name and telephone number
- Other Obligations: If you are required to make alimony or child
support payments, please include copy of Divorce Decree
- Application Fee: Your check of $350 made payable to The PrivateBank
will be applied toward your appraisal and credit report

verifications prior to closing.
Home Equity Checklist
To begin processing your home equity line of credit, please provide
the following:
- Home Equity Application
- Proof of Homeowner's Insurance
- Mortgage Statement (copy of your most recent statement for
existing home)
- Income Verification (includes pay stub, W2 or Tax Return)
